No. The app connects only to the module on your own server. We have no database anything could reach.
Reading the call state (to know who is calling), displaying over other apps (the popup) and internet access. We do not ask for contacts, SMS or location.
Yes. CallerPeek recognises calls on both cards; in the settings you can limit it to a chosen SIM.

In the module panel you revoke the old device token with one click and scan a new QR code on the new phone. The old phone loses access immediately.
